Are you a new Customer?
The following form will allow
you to establish new service within HC MUD 81. Fill it out and bring it to the District
Office and a customer service representative will assist you in setting up new service.
If you are unable to appear in person please contact the District Office for assistance.
What to bring:
- Completed
Establish Service Form
- Current Driver's License
- Signed Lease Agreement (if renting) or
- Closing Paperwork - HUD Settlement form (if purchasing)
A notarized Customer Service Agreement is required for all new accounts.
A one-time non-refundable $25 initiation fee is required and be on your first bill.
Are you moving?
The following form will allow you to discontinue
service within HC MUD 81. Fill it out and return it to the District Office.
What to bring:
*Please allow 2 (two) business for all requests.
*All forms can be provided by the district office.
